
Directed Donor Process
RECIPIENT PATIENT/COUPLE:
• Patient: Evaluation Cycle including Infectious Disease blood work (IDBW) on an annual basis.
• Partner (if applicable): Infectious disease blood work (IDBW) on an annual basis.
• Initial consultation with physician.
• Counseling with mental health professional as a couple (if applicable), and then with the directed sperm donor. Note to be provided to FCNE.
• Follow up consultation with FCNE physician and nurse to review treatment plan.
• Appropriate FCNE consents signed by recipient(s).
• Documentation of legal agreement between recipient(s) and directed sperm donor to be provided to FCNE.
• Frozen directed donor sperm to be shipped to FCNE for treatment cycle only after FDA clearance has been obtained.
• The sample must be logged in FCNE lab before medication can be ordered or testing scheduled (if not a medicated cycle).
• Stimulation cannot begin until sample is received.
Directed sperm donor:

Appointment with physician at Brown Urology to complete FDA physical, questionnaire and IDBW testing at Brown Urology.

Counseling with mental health professional alone and then with recipient(s). Both notes will be provided to FCNE.

Will utilize Brown Urology for all FDA required testing and semen sample production and cryopreservation. Sample is quarantined for at least 30 days and repeat IDBW is obtained after 30 days.

This office visit may include the screening process (Medical History, Questions for Potential Sperm Donor, and Donor Eligibility Physical Exam).

Pt may need to bring their first morning urine. If the patient is agreeable to starting the process as a directed donor, we may obtain bloodwork for communicable disease testing and a semen sample for cryopreservation.
